  The Truth About Food Cravings After Bariatric Surgery (And How to Beat Them)

Bariatric surgery, like the gastric sleeve or bypass, is often seen as a reset button for weight loss—but what if the cravings don’t actually go away? If you’re months or even years post-op and still hearing that relentless "food noise," you’re not alone.

Hi, I’m Shenelle, and I had my gastric sleeve surgery nearly three years ago. I took a strict approach from day one—completely eliminating problem foods and never reintroducing them. That decision made a huge difference. But not everyone follows the same path, and I’ve seen firsthand what happens when those old habits creep back in.

As a bariatric coach at The Rewrite Lifestyle Center, I work with individuals who are struggling with weight regain after surgery. One of the biggest culprits? Diets that still include ultra-processed “health” foods like protein shakes and bars loaded with artificial sweeteners. These products may look bariatric-friendly on the surface, but for many people, they stimulate the same cravings and dopamine rush that led to overeating in the first place.

Many post-op patients reach the 18-month mark and suddenly feel like all their old cravings are back—because in many ways, they are. The body’s hormonal cues eventually rebound, and if sugary or processed foods are reintroduced, the brain picks up right where it left off.

In fact, studies show that once sugar is eliminated and then reintroduced, your brain doesn’t forget the pleasure it used to get from it. The cycle of cravings, emotional eating, and food noise can restart.

The Answer: Abandon Moderation. Embrace a Whole Food Lifestyle.
For most people struggling with food addiction after bariatric surgery, moderation doesn’t work. That’s why I advocate for abstinence from problem foods—because when they’re gone, they’re no longer a problem. I know that sounds tough, but it’s doable. And you don't have to do it alone.
